Understanding the Medicare Part B Late Enrollment Penalty

Verified against SSA POMS as of July 27, 2026

The short answer

Medicare Part B Late Enrollment Penalty: The penalty for late enrollment in Medicare Part B is calculated based on the duration of delay in enrollment compared to the initial eligibility period. The penalty amount adds to the monthly premium and continues throughout the duration of your Medicare Part B coverage. Special enrollment periods exist where penalties may not apply.

Late Enrollment Penalty Explanation

The Medicare Part B late enrollment penalty is a financial charge added to the monthly Part B premium for individuals who did not enroll in Part B during their initial enrollment period (IEP) and are not eligible for a special enrollment period (SEP). The penalty is calculated based on periods of delayed enrollment and is applied throughout the individual's Medicare Part B coverage period. Specific exceptions and special circumstances may apply where penalties can be avoided.
The Part B premium is increased 10% for each full 12 months you delayed enrollment and lacked employer sponsored coverage. SSA only views penalties in 12 month increments. For counting purposes, the first late month is the month following the Initial Enrollment Period and the last month counted is the month that a beneficiary signs up for Part B.

Equitable Relief Options

Equitable relief may be considered for individuals who, due to government error or major disasters, missed their enrollment periods. Such relief can be granted on a case-by-case basis and may eliminate penalties if the request is made within 90 days beyond the normal enrollment period deadline.

Frequently asked

What happens if I enroll in Medicare Part B late?

Enrolling in Medicare Part B late may result in a late enrollment penalty, which increases the monthly premium for each year you delay enrollment without a special enrollment period. Penalties last as long as you are enrolled in Medicare.

How much is the Medicare Part B late enrollment penalty?

The Part B late enrollment penalty is an additional 10% of the Part B premium for each 12 months of delay in signing up. This penalty lasts as long as you have Medicare coverage. A delay between 24 and 35 months would mean a 20% increase in Part B premium. A 36 month delay would increase the penalty to 30%.

Can I avoid the Medicare Part B late enrollment penalty?

Yes, if you qualify for a Special Enrollment Period (SEP), you can avoid the late enrollment penalty. For instance, USPS employees have a specific SEP as mentioned in HI 00805.346. There are also weather related and natural disaster cases that can be made for equitable relief.

Do natural disasters impact my Part B enrollment deadlines?

Yes, natural disasters and weather related emergencies can extend the deadlines through equitable relief, allowing late enrollments to be considered timely as described in HI 00805.324.
